Exit Instruments for Intact and Reunifying Families

The National Family Preservation Network has developed exit instruments (surveys) for both workers and parents. The instruments are research based with positive findings. The instruments are completed at termination of services. The questions for both worker and parent(s) are similar.

 

For example, here is one question the parent(s) answers matched with one question for the worker:

The questions were also designed to correspond with the NCFAS Assessment Scales. When completed by both the worker and parent(s), the instruments show the interrelationship of assessment with family engagement, services, and outcomes. The optimal outcome is a high degree of alignment between the worker’s and the parent’s perceptions.
